	* kdpipeiodevice.cpp: Use namespace _gpgme_.
	[Q_OS_WIN32 && NOMINMAX]: Do not define NOMINMAX again.
	* w32-qt-io.cpp: Change namespace of KDPipeIODevice to
	_gpgme_::KDPipeIODevice.
