doc: Improve doc on passphrase_cb / pinentry mode

* doc/gpgme.texi (Passphrase Callback): Mention pinentry_mode and
restrictions.
(Pinentry Mode): Fix wording and clarify versions that need
loopback mode for passphrase_cb to work.
